Support Coordination

Level 2: Support Coordination

Support Coordinators can provide time limited, low level or specialist coordination of supports. Here are a few examples of the types of assistance a Support Coordinator can provide:

  • connecting you with informal, mainstream and funded supports to strengthen your capacity and resilience
  • negotiating services and pricing
  • arranging assessments required to determine the nature and type of funding required
  • liaising with Plan Managers to establish the appropriate claim categories
  • providing participants with assistance to: resolve problems, understand their responsibilities under service agreements
  • change or end a service agreement.

We will help you develop skills and confidence to take control of your NDIS plan. We aim to:

  • Develop your NDIS plan with you to best meet your needs and goals.
  •  Assist you to take over some Case Coordinator function over time, and to teach you how to request for things you need.
  • Once you are comfortable with the process, you can manage your plan independently. This is an individual process and everyone learns these skills in their own time.

Stages of Support Coordination:

Your first meeting with Forward Focused will be with our Relationship Manager. We will walk through your NDIS plan and assess how we can best support you in achieving your NDIS goals. We will encourage you to share information about yourself i.e. name, NDIS number, disability and people most important in your life, so we understand what goals you would like to achieve.

During this meeting, a Service Agreement will be developed. This outlines how Forward Focused and you will work together. If you decide Forward Focused is your chosen provider for Coordination of Supports, you will need to sign the Service Agreement. We will also go through, complete, explain and sign consent forms, privacy & confidentiality form and an individual client assessment form.

The Relationship Manager will then match you with the best Support Coordinator to meet your needs, goals and aspirations. Next, your dedicated Support Coordinator will contact you and start your NDIS Journey!

Your matched Support Coordinator will contact you and coordinate a meeting. Your circle of supports are encouraged to attend this meeting to help compile a comprehensive picture of your life.

During this meeting we will talk about how to meet your NDIS goals, understanding your funded support categories and explain the do’s and don’ts of Support Coordination. Together we will develop a unique, tailored and whole-of-life program that reflects your specific needs, desires and aspirations. Therefore enabling us to achieve the goals in your NDIS plan.

After the meeting, we will complete a profile so we can personalise your service and match you with the right support staff and services for you.

Your Support Coordinator will meet with you to determine how your goals are being achieved and will assist you in navigating the NDIS through connecting and coordinating informal, mainstream and funded supports in a service delivery environment. We will walk alongside your NDIS journey and are committed to help ‘make it happen’.

We will provider you with a brief progress report and list of services connected or to be arranged after 6-8 weeks of service.

We will also collect some feedback from you, your family and carers during this time so that we are best positioned in continuing the right supports and service delivery excellence to you.

Once your supports and services are established, together we will monitor your goals, needs and aspirations.

We will keep records about how goals are being achieved so there is good information available for you to use when you review your plan with the NDIA. This usually happens once a year.

As part of the process of working with you, we aim to develop and strengthen your skills, capacity and confidence in taking control of your NDIS plan. Once you are comfortable with coordinating your NDIS plan, you have the choice to independently manage your own plan.

We will also collect some feedback from you, your family and carers during this time so that we are best positioned in continuing the right supports and service delivery excellence to you.
